School Nurse and Health Information

School health legislation requires that all 10th graders have a complete physical exam before the beginning of 11th grade.  If written proof of a physical exam is not submitted by the end of their sophomore year, they will not be permitted to re-enter school the following year (junior year).

Students in grades 6-12 participating on a school sports team must submit a complete physical exam form to participate. These physicals are valid for 13 months.

Connecticut State Law and Regulations 10-212(a) require a written medication order from an authorized prescriber and parent/guardian written authorization, for the nurse, or, in the absence of the nurse, a designated principal or teacher, to administer medication. Medications must be in the original, properly labeled container and dispensed by a physician/pharmacist.
