In order to participate in athletics at Lewis Mills High School,
student athletes must:
1) have an updated physical
exam on file with the school nurse and
2) be registered each
athletic season on FamilyID.
Physical Exams
Physical exams must be submitted to the Lewis Mills School
Nurse's office prior to participation in a sport (they are not given
to the coach). Physical exams must be current - they are valid
for 13 months from the date of the physical - and must be signed by the
physician. The form needed can be
accessed here.
The student must visit the nurse's office to obtain a Blue
Card (which indicates the athlete is cleared for entire season) or
a Red Card (cleared to start the season but updated physical
needed) that they then must give to the coach before the first
try-out or practice. If the physical examination expires during
the course of the season (as indicated by a Red Card), a new exam must be completed
and submitted to the nurse in order to be allowed to continue
participating after the expiration date.
If a player shows up to the first day of tryouts without having
obtained a blue or red card from the nurse or shows up with their physical form
in hand, they will NOT be permitted to participate.
Registering Using FamilyID
All Lewis Mills Athletic registration is now completed
through FamilyID (https://www.familyid.com/organizations/3586/programs). The profile you
create in FamilyID keeps track of the information entered so that it is
completed only once for multiple uses, multiple family members, and
multiple programs. At the beginning of each sport season you will
register your child using the FamilyID registration link for that season.
If this is your first time registering with FamilyID, it will be
helpful to have the following information handy to allow for accurate
completion of your online registration:

Insurance information
- Physician's
name and phone number
- Name
and Phone number of Emergency Contacts
