Athletic Links and Resources

  • Captain's Application (pdf) - Student-athletes interested in becoming a team captain should apply using this application.  Completed applications should be submitted to the Director of Athletics, who will review and sign it before forwarding it to the varsity coach.  A Microsoft Word version can be found here.
  • Alternate Transportation form - Student-athletes are required to travel to and from sports events on the bus provided by the school.  Should a need arise where a student-athlete needs to drive themselves or a parent needs to drive other athletes, this form may be signed by the parent and given to the principal or athletic director for approval.  The form will then be given to the coach so they are aware of the ride arrangement.

Note: This form is not required for a parent to pick up their own child at an away event; the parent can email the coach directly (please give 24-hr notice).  The parent is asked to make visual contact with the coach upon departing the event with their child. 

  • CIAC Concussion Information - During the 2014 legislative session the Connecticut Legislature modified the laws regarding concussion education to include a plan that would educate student-athletes and parents, as well as require each parent and athlete to sign an informed consent form.
  • NCAA Eligibility Center - The NCAA Clearinghouse is an essential step in becoming eligible to play college sports. Over 180,000 potential college athletes register with the NCAA every year. If you want to play NCAA college sports and receive a scholarship at the DI or DII level, you will need to register and be cleared by the NCAA. The Eligibility Center is the organization within the NCAA that determines the academic eligibility and amateur status for all NCAA DI and DII athletes.  A comprehensive guide for the college-bound student athlete can be found here.
