Lewis S. Mills  Athletic  Registration

To be eligible to participate in Lewis S. Mills's Athletic Program, students must: 
1. Have a valid Physical Examination on file in the School Nurse's office.
2. Have fulfilled all Academic Eligibility Requirements. 
3. Register with Family ID at Lewis S. Mills 's Family ID webpage 
4. Football Registration deadline is August 13th.  All 
participants must have completed an electronic registration on-line at Lewis S. Mills FamilyID webpage
5. All other fall sports Registration deadline is August 23rd.  All participants must have completed an electronic registration on-line at Lewis S. Mills FamilyID webpage


Physical Examination Requirements:  
Physical exams must still be submitted to the Lewis S. Mills School Nurse's office prior to any participation.  A physical exam must be current (within the past 13 months) and signed by the physician. Students STILL will receive a Blue Card or a Red Card and will hand that card to the coach on the first day of practice.  If the physical examination expires during the course of the sports season, the athlete will be required to be examined prior to the date of the physical examination's expiration, otherwise continued participation is prohibited.

Lewis S. Mills High School Academic Eligibility Requirements:

A student cannot at any time represent Lewis S. Mills High School unless he or she is enrolled in at least four (4) quarter Carnegie Units of work or its equivalent.

For fall sports eligibility, a student must have received credit toward graduation at the close of the school year preceding the contest in at least four (4) Carnegie Units of work or its equivalent for which he or she has not previously received credit. Credit must be earned during the same academic year. A student enrolling in ninth grade for the first time will be eligible for the first grading period regardless of the previous academic achievement.

During the school year a student must have received a passing mark in at least four (4) quarter Carnegie Units of work or its equivalent at the end of the regular marking period preceding the contest. Student eligibility will be determined for all students on the date that report cards are distributed or on the fourteenth calendar day following the end of the marking period, whichever comes first. No Carnegie Unit or equivalent for which the student has already received credit shall be included in those required by this rule.


FORMS TO BE COMPLETED USING FAMILY ID  BEFORE YOU CAN TRY OUT:

Athletic registration for Lewis S. Mills  High School Athletic Teams takes place through the services of our partner company "FamilyID".  Paper "Permission Forms" and "Emergency Information Forms" are no longer used or accepted.  Instead, registration occurs online via Lewis Mill's Family ID webpage, which keeps track of the information in your FamilyID profile.   Information is entered only once for multiple uses, multiple family members, and multiple programs throughout the duration of your high school athletic career.

2016-2017 Athletic Dates:

Specific Times and Locations for tryouts will be posted on the Athletic website prior to the start of each season. Information will also be announced during morning announcements.

Fall Season: Soccer, Cross Country, Crew, Girls Volleyball, Field Hockey– Begin August 25, 2016 

Football: Begins August 15, 2016
Cheerleading: The coach will post tryout information on in early Fall.
Unified Sports:  Will meet with students when schools begins in September

Winter Season: Boys Basketball, Boys & Girls Indoor Track, Swimming, Ice Hockey, begin December 3, 2016; Girls Basketball begin November 28, 2016

Spring Season: Golf, G/B Tennis, Crew, Baseball, Softball, B/G Lacrosse, B/G Outdoor Track begin March 18, 2016; Baseball Pitchers and Catchers start March 13, 2016.
