Athletic Registration is New for the 2016-2017 School Year!!!!!

Athletic registration for Lewis S. Mills  High School Athletic Teams takes place through the services of our partner company "FamilyID".  Paper "Permission Forms" and "Emergency Information Forms" are no longer used or accepted.  Instead, registration occurs online via 
Lewis Mill's Family ID webpage, which keeps track of the information in your FamilyID profile.   Information is entered only once for multiple uses, multiple family members, and multiple programs throughout the duration of your high school athletic career.


Physical exams must still be submitted to the Lewis S. Mills School Nurse's office prior to any participation.  A physical exam must be current (within the past 13 months) and signed by the physician. Students STILL will receive a Blue Card or a Red Card and will hand that card to the coach on the first day of practice.  If the physical examination expires during the course of the sports season, the athlete will be required to be examined prior to the date of the physical examination's expiration, otherwise continued participation is prohibited.

It will be helpful to have the following information handy to allow for accurate completion of your online registration:

Health Insurance Information, Physicians name and number, name and phone number of emergency contact

A parent/guardian should register by visiting Lewis S. Mills's Family ID page and following these steps:
1.      Under the "Programs" title, click on the link for the program you want to sign up for. Scroll to bottom of page to either Sign Up as a Family or Individual if this is your first time using FamilyID, or Log In if you already have a FamilyID account.
2.      Sign Up for your secure FamilyID account by entering your family name, email address and password. You will receive an email with a link to confirm your new account. (If you don't see the email, check your spam or junk folders.)
3.      Click on the link in your email confirmation, which will log you in to FamilyID, and then complete the information requested on the registration form. When you have completed the form, click the "Save & Continue" button.
4.      Review your registration summary and click "Confirm Registration" to submit. Once completed, you will receive an email receipt from You can also view your completed registration on your Dashboard. You can log into at any time to update your information and to check your registration(s).

Thank you to the Cheer Team for their contributions during the fall season.  We struggled to find a coach for the season and Lindsey Small stepped in and supervised the girls. LSM appreciates the girls effort, determination, enthusiasm and school spirit as they cheered for the football team!

Dear Parents, Guardians. Students, Staff and Community Members:

The Lewis S. Mills Athletic Department and the administration of Lewis S. Mills High School would like to invite you to an informational meeting regarding a potential move in athletic conferences. 

Currently, our athletic teams compete in the Berkshire League, Pequot League and Western Connecticut Lacrosse Conference. We are considering applying to the Central Connecticut Conference for the Fall of 2017.

The presentation will take place in the Lewis S. Mills auditorium Tuesday,  December 6, 2016 at 7:00. I hope and encourage you to attend this evening event at Lewis S. Mills.

Following a presentation
consisting of the following discussion points our panel will answer questions from audience members.

  1. JV Programs will have a complete schedule for all teams
  2. Better Competition to prepare our teams for state tournament
  3. Transportation cost will be go down
  4. Playing a diverse group of schools that offer all programs we have
  5. Great recognition and for athletes that plan to participate in sports in college
  6. Tradition - roots of the old NWC
